On most domestic roofs the rubber goes down as a single sheet with no field seams at all. Seams are where flat roofs fail, so a roof with none of them is a roof with far less to go wrong.

Sometimes, if the existing build-up is sound, dry and firmly fixed. More often the honest answer is that stripping it off is the only way to know what the deck is doing, and covering a wet deck just hides the problem.
For maintenance access, yes. If it is a balcony or a regularly used terrace it needs a walkway or a protection layer designing in, and that gets discussed before it is laid rather than afterwards.
